How to Submit a Form
Submit forms using the online submission links provided in the sections below unless otherwise directed.
For urgent incidents: Contact PGCC Campus Policy/Public Safety immediately before submitting documentation. See College Safety.
Incident Reports
Report any employee, student, visitor, or property incident using the PGCC First Incident Report Portal.
- Notify your supervisor

The supervisor should complete the PGCC First Incident Report
- Preserve evidence when safe (photos, damaged equipment, witness information). Attach documentation to the report.
