How to Submit a Form

Submit forms using the online submission links provided in the sections below unless otherwise directed.

For urgent incidents: Contact PGCC Campus Policy/Public Safety immediately before submitting documentation. See College Safety.

Incident Reports

Report any employee, student, visitor, or property incident using the PGCC First Incident Report Portal.

  1. Notify your supervisor
  2. The supervisor should complete the PGCC First Incident Report

  3. Preserve evidence when safe (photos, damaged equipment, witness information). Attach documentation to the report.
